Volunteer/Employee Risk Management 2010 Instructions:

 New for 2009 - all volunteers will automatically have a background check run once thier application is submitted. SoccerMaine will manage background checks on our behalf. You will need to answer additional questions and check the box giving your permission for the background check.

For completion by adult participants of Soccer Maine Member Clubs whether coach, assistant coach, substitute coach, staff coach, skills trainer, director of coaching, associate director of coaching, athletic trainer, program administrator, referee, referee assistants, fourth official, assessor, instructors, certified referee assignor, ODP/State Select coaches and administrators, and Club officer employed, serving as a volunteer of, serving as an independent contractor of, or endorsed by a Club or a team of a Club (each such person being 'associated' and having an 'association' with such Club)

You MUST submit one Risk Management application each year. If you have already done so, and have additional positions (another club, or another position in your club that you forgot to list on the first application, or both) that you must add to your submitted application, please select 'I want to add another volunteer position'.

Next select your status for registration. Your choices are:

• I registered in 2008/2009. This is my first registration in 2009/2010.

This is for all returning volunteers who signed up in 07-08 & 08-09.

• I have never registered before. I am new to Soccer Maine.

This is for all new volunteers or those who have never used this system.

• I want to change some of my personal information on file in 2008/2009.

This is for anyone wishing to change contact information for 09/10.

• I want to add another volunteer position.

This is for anyone in the system that needs to add extra positions.

An example of this is a volunteer who coaches multiple teams.

Now click the Online Soccer Maine Risk Management Application button.

Complete the form, adding information to any field marked with an asterisk (required). Be sure to include an email address in section III. Mass email will be one of the primary tools of this system and without a valid email address you will miss out on a lot of the Soccer Maine communication attempts. After completing the form, hit the Submit Risk Management Information button.
